Advance Directives

Advance directives are instructions (verbal or written) about future medical care. The directives allow you to identify your wishes for treatment and name a person to make decisions on your behalf if you are unable to speak for yourself. Examples of advance directives include: Living Will, Power of Attorney, Health Care Representative, and Out of Hospital Do Not Resuscitate Declaration and Order.
