| CMS Merit Award Kosciusko Home Care & Hospice, Inc. Presented Award of Merit Warsaw, IN.—Health Care Excel, the Indiana Medicare Quality Improvement Organization (QIO) honored Kosciusko Home Care & Hospice, Inc. for meeting established criteria for the Medicare QIO’s Home Health Quality Improvement Award of Merit and providing quality health care to the Medicare beneficiaries they serve.
